Consultation Payment Agreement With Dr. Ofer Zur
This page provides you with information regarding Dr. Zur’s consulting and psycho-educational services, explains the terms of these services and enables you to pay for the services.

- Phone and in-person consultations regarding clinical and/or ethical matters are charged at the rate of $200.00/half hour, $275.00/45 min or $350.00/hour.
- The consultations and psycho-educational services described on this page neither constitute psychotherapy or counseling, nor involve mental health diagnosis, evaluation, or treatment, nor render legal advice, nor constitute supervision.
- Dr. Zur will refer you to a licensed mental health professional/s if he determines that psychological or mental-health intervention, treatment or assessment is needed.
- Dr. Zur will keep the consulting records for 7 years.
- Because the standard of care is complex, context based, and multidimensional, in many situations it is advisable that you seek legal consultation from an attorney or additional ethical/clinical consultation/s from other experts.
- Some circumstances requiring disclosure by law include a reasonable suspicion of child, dependent or elder abuse or neglect or other situations required by law.
- Payments are due at least 24 hours prior to the consultation.
- Reading material and review of records will be charged at the same rate as the phone or in-person consultation.
- Unless cancelled 24 hours before a scheduled appointment, your credit card will be charged at the half hour rate.
- It is understood and agreed that the payment is non-refundable, unless agreed otherwise, involves neither a written report nor a follow up, and is in no way contingent upon the outcome of the consultation.
- There is no guarantee that Dr. Zur’s expert opinion will support your claims, views, attitudes, opinions or concerns.
